About our team : The Access Mental Health team sits alongside and compliment those provided by primary and secondary care. This is all about GP surgeries, hospitals and the third sector working together so anybody who needs a service sees the right person at the right time, through a single point of entry.

Together, we’re making sure we’re walking alongside people who need support every step of the way, making sure they are ok, making sure they’re accessing relevant support and then thinking about their next steps.

How will you make a difference?

As a Community Wellbeing Worker you will be the first point of contact for people coming into the service and will be part of a collaborative team supporting people and walking with them through the journey of accessing the services that they need. You will walk with people from journeys beginning to journey end, this includes providing assessments, information and develop individual plans alongside holding a caseload.? You will provide Management to Wellbeing Peer Workers and will work collaboratively with the team and have regular communication with the Service Manager.?

You will also promote the service by linking in with external agencies to build relationships and promote the service offer, coordinate referrals into the service, and liaising and network with relevant statutory and third sector organisations. You will also engage with community events, meetings and forums to promote the service and Rethink Mental Illness whilst encouraging community to find out about local services and facilities and enable access to alternative services where specific needs are identified.

Your role will be a field based position where you will work flexibly whilst covering the geographical area of Wiltshire and working from home on average one day a week. You will work Part Time, 21 hours per week on a permanent contract with a salary of £26,871pa. The role includes some evening and weekend working. What we are looking for : You’ll have the essentials covered : Ability to make a comprehensive assessment of an individual’s situation and needs; to develop, implement and review risk management plans and devise support plans and activities to meet these needs.

Have experience of supporting people experiencing mental health crisis and those with complex emotional needs who are at risk of crisis.

Have experience of partnership working, negotiation and liaison work with other agencies

Have experience of staff supervision and coaching

Have excellent written and verbal communication skills, tailored to a variety of audiences

Be proficient, adaptable, trauma-informed

Have ability to work flexibly and proactively with partners
