Job Description 3 months contract with a North London based Local Authority Job Summary: ·          We are looking for individuals who are passionate about working with people who have learning disabilities and their families. ·          If you are committed to supporting people with learning disabilities to live autonomous lives and achieve positive wellbeing, then we want to hear from you. ·          This is an exciting opportunity to be based within a local authority integrated health and social care service. ·          The service comprises of many disciplines including Social Work, Community Nurses, Psychologists, Occupational Therapists, Speech and Language Therapists, Art Therapists, Physiotherapists and Psychiatrists. ·          The Care Coordinator post will sit within the social work team. Key Duties/Accountabilities: ·          The role entails face to face work with service users, their families, and external partners. ·          The purpose of the Care Coordinator role is to identify whether support packages continue to meet the needs of service users and their families. ·          The role will require the successful candidates to make recommendations regarding support packages and arrange and amend support to ensure needs are being met in a person centred and strength-based way. Essential Experience Required: ·          You will need excellent communication skills, both verbal and written. ·          The Care Coordinator will be required to attend multi- disciplinary meetings and present cases, communicate effectively with service user’s and their families as well as a range of internal and external colleagues and partners. ·          In addition, the Care Coordinator will be required to write accurate, comprehensive support plans and assessments.
