Care Coordinator - Children's Complex Care - Band 5

We are looking to recruit a new Band 5 Care Coordinator to work alongside our existing team of Care Coordinators. The Care Co-ordinators have a management and clinical role within the Children's Complex Care Team. Work across Gloucestershire in a countywide capacity to deliver care in children’s home during both days and nights, as required. The shifts required will include some weekend working. TheCare Coordinatorwill undertake line management of the clinical support workers in the Children’s Complex Care Team and also have a caseload of patients that they will be a professional lead for in the service. TheCare Coordinatorwill support the Team Leader for the Childrens Complex Care Team in undertaking audits, supporting the lone working systems at night and taking on a champion role in the team in a set area (Manual handling, safeguarding, care plans, staff rosters etc) TheCare Coordinatorwill be the named contact for general queries for a caseload of children. The role of the Children’s Complex Care Team is to support children with life limiting and life threatening conditions in their own home. TheCare Coordinatorwill work as an integrated member of the Children and Young People’s community team in Gloucestershire; providing home-based support and respite care to children with life-limiting conditions. TheCare Coordinatorwill also undertake a range of delegated patient duties in the community, but have the ability to act on their own initiative in delivering patient care. Using a holistic approach theCare Coordinatorprovides care and support to parents and siblings to meet the individual needs of child/young person within a community setting. TheCare Coordinatorwill provide a safe, stimulating and inviting therapeutic environment, which fulfils the developmental and social needs of the child and family. TheCare Coordinatorwill undertake a leadership role in the team, acting as direct point of contact for support workers, raising issues as required to the Team Leader for Children’s Complex Care Team. TheCare Coordinatorwill hold a caseload of children whom they will be the named lead professional in the team for.
