Main duties of the job Following a comprehensive induction programme, you will undertake a rotational clinical pharmacist role working closely with pharmacy colleagues and the wider multidisciplinary team. You will engage directly with your patients to achieve medicines optimisation throughout their inpatient journey from medicines reconciliation through to discharge. Rotations include a variety of specialities including stroke, frailty unit, paediatrics/neonatal, emergency department, healthcare of the older person, acute admissions unit and post-operative surgery. In addition to providing a pharmacy service at ward level, you willalsoundertake the role of responsible pharmacist when working in outpatient areas on a rotational basis.We will provide all the resources you need to help develop your knowledge and skills during each rotation, providing support from senior clinical pharmacists, rotation objectives and study time to complete diploma work. We are continually looking to improve and develop our team and services, so all our clinical pharmacists are encouraged to contribute to our Clinical Improvement Group and Health and Wellbeing groups. Job description Job responsibilities Through a varied rotational programme, to work within multidisciplinary healthcare teams to provide ward based clinical pharmacy services.To deliver medicines optimisation and the safe transfers of care around medicines.To participate in weekend, on-call, Bank Holiday, early and late working in accordance with agreed rotas.To undertake a post graduate diploma in clinical pharmacy.To undertake necessary continuing professional development in line with General Pharmaceutical Council standards.To provide clinical pharmacy support to the inpatient and outpatient dispensaries and to undertake the role of Responsible Pharmacist when required.To participate in the provision of medicines information and advisory services to patients and their relatives, medical and nursing staff, other healthcare professionals and organisations.To provide professional supervision to pharmacy technical and support staff.To contribute to the training of trainee and undergraduate pharmacists and pharmacy technicians.To participate in clinical audit.To contribute to the practice and development of quality governance throughout the department.To undertake any identified clinical and professional training and actively participate in the staff appraisal process.To maintain professionalism and confidentiality at all times.To be familiar and comply with relevant operational, personnel, health and safety policies and procedures, including fire safety, infection control, COSHH, no smoking and alcohol. Person Specification Qualifications Essential qualification Registered with the GPhC Desirable Membership with other relevant professional bodies UKCPA, RPS Experience Essential Insight in to the practice of hospital based clinical pharmacy Desirable Experience of hospital practice at pre-registration level Skills and Knowledge Essential Good grasp of the principles of clinical pharmacy / therapeutics Knowledge of hospital pharmacy practice Knowledge of modern approaches to Medicines Management / optimisation Evidence of CPD Desirable Extended knowledge of hospital pharmacy practice and Medicines Management / optimisation Personal Attributes Essential Confident communicator Energetic and enthusiastic Strong team skills Self-directed and able to work independently Capable of undertaking Post Graduate study Ability to prioritise and work under pressure CARE values Essential CARE values
