This is an exciting opportunity to support vulnerable adults living in their own homes in the community, ensuring that they can stay at home during times of poor health or other emergencies, and to support them, where appropriate, to build their skills and levels of independence back up through the achievement of smart and measurable goals. This is a valuable service, making a real difference to our local communities while supporting local health services to maintain capacity by reducing the need for people to stay in hospital if they can be supported safely and effectively at home through your caring and nurturing approach. Supporting vulnerable people to live safer and supported in their own homes, you’ll follow individual Short Term Care at Home Plans, working with people to maximise independence where possible. This will include supporting with activities such as personal care (bathing, dressing, managing continence needs, medication support), shopping, mealtimes and light housework where needed. You’ll be given all the training and mentoring you need to ensure you are fully equipped to complete the role to the best of your ability and make a real difference to the people of Lancashire. You will be asked to complete a robust training programme, with a blend of face-to-face training (paid), e-learning, and on-the-job mentoring, all combining to ensure you meet the standards of the Care Certificate.
