Clinical Nurse Specialist - Dementia Outreach Team

The post holder will be based within the Dementia Outreach Team who are currently based in Airth GP Practice. This post is for 37 hrs. The aim of the Dementia Outreach Team is to provide an intensive intervention to people with a confirmed diagnosis of dementia and their carers when they are in, or at risk of entering a crisis situation. As part of the Dementia Outreach Team the post holder’s role will include working collaboratively with health and social care professionals in primary, secondary and third sector community settings to meet the mental health needs of people living with dementia and promote patient centred care across the Forth Valley area. The suitable candidate should have experience in working within an older adult mental health setting and possess sound knowledge of dementia and associated conditions. Excellent communication skills, time management and ability to work autonomously is essential. You should be a registered mental health nurse educated to degree level. Post graduate qualifications relevant to this area, or willing to work towards is desirable. Ability and willingness to travel throughout the area of service delivery is essential for this post.
