Required to visit patients at home for venepuncture

Crawley Care Collaborative are part of a wider Primary Care Network (PCN) which is a multidisciplinary network, combining community nursing and proactive care teams with other professionals. PCNs are the core element of a wider system responsible for coordinating care delivery to patients on the caseload. Care is organised around individuals' identified needs through collaborative working by service providers.

The post holder will need to be an experienced phlebotomist with excellent communication skills.
