Working in partnership with the NHS continuing healthcare teams, our enhanced level of specialist clinical support enables individual clients to remain at home even when the level of health care required is highly complex. Our services are provided through local and specialty focused multi-disciplinary teams. As an  Adult Nurse Case Manager (Tracheostomy Experienced),  you would be responsible for providing face to face support to our client and their families whilst creating personalised care plans for each client. You would work closely with a core team of carers training and supporting them with clinical skills and activities.
